What the affiliate tiers are measured on
The programme has two different ladders running at once, counted on two different things. Confusing them is the fastest way to misread what you are owed.

The affiliate programme pays on two axes, and they are not the same axis. Your commission rate is set by one of them and your monthly bonus by the other. Partners who read them as one ladder tend to be surprised in both directions.
Rate is counted on qualifying sales
The tier you sit in, and therefore the percentage you are paid, is decided by the number of qualifying sales attributed to you. It is a count of purchases, not a sum of money, and a run of smaller accounts moves you up the ladder exactly as fast as the same number of larger ones.
Bonus is counted on volume
The monthly bonus bands are measured on the value of sales over a rolling thirty days. Here the opposite is true: a handful of large accounts can reach a band that a larger number of small ones does not. The bonus sits on top of commission rather than replacing it.
